Injury Reporting and Insurance Claims

Injury Reporting
The Association requires injuries suffered in a game or at training be notified to it within 72 hours. For this reason at every soccer ground there are Accident Report Forms, which should be completed by the coach or manager of any player who is injured whilst playing soccer. The form is handed to Ground Control and is returned to the Association with the match cards at the end of the day’s play. The Accident Report Form must be signed by an official or the referee.
If you suffer an injury at training and there is no access to an Accident Report Form (available at the Canteen), please contact a Committee member, so a form can be completed and forwarded to the Association. Even if you feel the injury you have suffered is minor, complete an Accident Report Form, to do so will cover you if the injury proves to be worse than originally thought.
Insurance Claim
In some cases an injury will lead to the submission of an insurance claim to cover medical expenses and loss of income. If you need to submit an insurance claim you will need to;
1. Click here to download and complete the QBE Football Sporting Accident Claim Form.
2. Have the Sections on the Football Sporting Accident Claim Form completed by;
(a) The Player
(b) Your Doctor
(c) Your Employer
(d) Your Club (A Redbacks Club Official)
3. After this is done the Football Sporting Accident Form is forwarded to the Sutherland Shire Football Association for submission to QBE insurance. The insurer will contact you directly from that point.
